24-Hour Affidavit
Drivers age 21 and older who fail their first driving test and choose to complete 24 hours of supervised driving instead of 4 hours of behind-the-wheel instruction with a licensed driving instructor must document their practice hours using this form. A 4-hour approved adult driver training course is also required. The completed affidavit must be notarized before retesting.

50-Hour Affidavit
The 50-Hour Affidavit (Ohio BMV Form 5791) is required for drivers under age 18 before taking their driving skills test.
This form verifies that the student has completed at least 50 hours of supervised driving practice, including 10 hours of nighttime driving, with an eligible adult.
The form must be signed and notarized before submitting it to the Ohio BMV.
